Peripheral T-cell lymphomas (PTCL) are a group of rare aggressive non-Hodgkin’s lymphomas with an unfavorable prognosis. The 5-year overall survival rate is 30–50% (with the exception of ALK-positive anaplastic large-cell lymphoma, at 77–85%). Due to high hematologic toxicity and efficacy comparable to CHOP, antifolates, alemtuzumab, hypomethylating agents, histone deacetylase inhibitors, and lenalidomide have not been adopted as first-line therapy. Modern first-line therapy regimens (CHOP, CHOEP, CHP-Bv, CHEP-Bv) are based on the results of a randomized phase III trial ECHELON-2, which demonstrated the advantage of Bv-CHP over CHOP in CD30+ PTCL, as well as a number of large population-based retrospective studies, who showed the potential benefit of adding etoposide to CHOP. New data on the biological heterogeneity of PTCL may influence the choice of therapy in the future. It was found that PTCL from T-follicular helpers are more sensitive to hypomethylating agents and JAK inhibitors. The evidence base for the effectiveness of brentuximab vedotin in low or negative CD30 expression is growing. This article is devoted to the main first-line chemotherapy regimens and possible future treatments for PTCL, taking into account new data.
