Diabetic peripheral neuropathy (DPN) is a leading cause of nontraumatic amputations of lower extremities. The present article is concerned with the role of hyperglycemia in the development of microvascular complications of diabetes, including DPN, and mechanisms of tissue damage due to hyperglycemia. The data for the clinical picture and the principles of diagnosis of DPN, as well as contemporary approaches to the treatment of diabetic complications with the use of symptomatic and pathogenetic agents are presented. The latter include, in particular, thioctic acid and benfotiamine.
